Forward2025 Building Inova Care
Course Description:
Building Inova Care is a recurring 16 week series that takes physicians, advanced practice providers, nurses and other allied health professionals through the development of Care Models and Care Pathways, the evidence & experience-based way in which care is delivered and how optimized teams work together to create a seamless end-to-end patient journey. Teams will meet weekly for 60-120 minutes and work to gather and implement current guidelines and other evidence sources to build a comprehensive Care pathway that spans from disease identification through optimization/treatment and return to primary care.
Learning Objectives
Learning Objective 1:
Increase understanding of Inova Care and apply this to creating holistic Care Models and Care Pathways to support its delivery
Learning Objective 2:
Apply a standard framework to scope the work to develop a Care Model or Care Pathway and begin to consider how different processes and resources could support the care delivery in the framework
Learning Objective 3:
Apply knowledge about optimized care teams to build a Care Pathway Development Team and assemble a care team to support a specific Care Pathway
Learning Objective 4:
Develop a set of metrics to assess Care Pathway processes and outcomes
