Objectives:

• Increase understanding of Inova Care and how Care Models and Care Pathways support its delivery
• Learn about the Inova Care Playbook and resources to support the development of Care Models and Care Pathways
• Learn about the framework to scope the work to develop a Care Model or Care Pathway and begin to consider how the different TEAMs models would fit in the framework
• Learn about the optimized Care Team initiative and begin to consider who will be included in the specific Care Development Teams
